Procedures and Forms
All children who wish to join the Walking Bus will need to complete some forms for insurance purposes. The easiest way to manage this is to put together a registration pack. Example forms are available here and may be copied by schools and Walking Bus Co-ordinators. You can adapt the forms if necessary and maybe add your own school’s logo.

The documents (file sizes shown in brackets) are in Portable Document Format (PDF) which requires Adobe Acrobat Reader. If your computer does not have this software, it may be downloaded from here.
Volunteers should also be given a copy or summary of the route assessments made by Road Safety Officers.
These forms were compiled by Safe-Kids-Walking but may be photocopied and used by schools and parents to help organise Walking Bus schemes. Some of the content was based on the Department of Transport’s School Travel Plan Guidelines. Safe-Kids-Walking tries to be as accurate as possible but cannot accept liability for errors. Make sure your local authority and insurers accept use of these forms.
